I was using a logical AND where I should have been using an OR.
I have no idea why I overlooked this so long. When I saw this a few minutes ago I had to scold myself :-)
I can't count the number of times AND and OR have bitten me, LOL. Couple those with a few NOTs and misplaced ANDs and ORs get really fun to track down. It happens (and will again in my experience).
